A Declaration of Paternity (VS-8) allows unmarried parents to voluntarily establish the legal paternity of a child and have the father’s name added to the child’s Kentucky birth certificate.

The Bullitt County Health Department can assist parents with completing a Declaration of Paternity. An appointment is required.

What to Bring

Both parents must be present and must provide acceptable identification. Documentation for the child is also required.

Accepted Identification for Parents

Each parent must provide one current photo ID from the following list:

Driver’s License
State-issued photo ID
U.S. Passport
Military ID
Permanent Resident Card
Employment Authorization Card
Current school photo ID

Accepted Documentation for the Child

Please provide one of the following documents for the child:

Perinatal Hepatitis B Form
Infant’s Hospital Bracelet
Hospital Certificate of Birth
Discharge Paperwork

Completing the Declaration

The Declaration of Paternity must be completed correctly and signed by both parents in the presence of a notary. Bullitt County Health Department staff will review the form, verify the parents’ identities, and assist with the required notarization.

Parents will also receive an explanation of their rights and responsibilities before signing the Declaration.

Important Information

A Declaration of Paternity is a legally binding document and has the same weight and authority as a court-ordered determination of paternity.

Parents should carefully read and understand their rights and responsibilities before signing. If there is any question about paternity, either parent has the right to request genetic testing before signing the Declaration.

Either parent may request to rescind the Declaration within 60 days of the notarized signatures, subject to applicable Kentucky requirements.

Special circumstances may require a different process. For example, additional requirements apply when the mother was married to someone other than the biological father at the time of the child’s conception or birth.

Patients may request copies of their medical records from the Bullitt County Health Department by completing a Medical Records Request Form.

To help protect patient privacy, medical records will only be released in accordance with applicable federal and state privacy requirements. Additional identification or authorization may be required before records can be released.

The Bullitt County Health Department is committed to providing access to public records in accordance with the Kentucky Open Records Act, KRS 61.870 through KRS 61.884.

Many records maintained by the Health Department are available for public inspection or copying. Certain records are exempt from disclosure under state or federal law, including protected patient and health information subject to HIPAA and other applicable confidentiality requirements.

How to Submit a Request

To request copies of public records or to inspect records maintained by the Bullitt County Health Department, please complete the Open Records Request Form and submit it using one of the following methods:

Copies of available records are $0.25 per page.

Response to Your Request

In accordance with the Kentucky Open Records Act, the Bullitt County Health Department will provide a written response within five business days of receiving an Open Records Request, excluding Saturdays, Sundays, and legal holidays.

The response will indicate whether the request can be fulfilled or whether any portion of the request is denied or exempt from disclosure.

If responsive records are in active use, in storage, or otherwise unavailable, additional time may be necessary. In these circumstances, the Health Department will provide a written explanation for the delay and identify the earliest date the records are expected to be available.

Requests involving a large number of records, records requiring review or redaction of confidential information, or records maintained in multiple locations may require additional processing time.

The Bullitt County Health Department provides certain services related to Kentucky vital records as part of the state's vital statistics program.

Services Available at BCHD

The Bullitt County Health Department provides:

Declaration of Paternity (VS-8) – by appointment only
Vital statistics forms and instructions for funeral homes, hospitals, nursing and extended-care facilities, hospice organizations, and the Coroner's Office, as applicable
Applications and information for requesting certified copies of Kentucky birth, death, marriage, and divorce records

Certified Birth, Death, Marriage & Divorce Certificates

Certified birth, death, marriage, and divorce certificates are not issued by the Bullitt County Health Department. Certified copies are issued by the Kentucky Office of Vital Statistics (OVS) in Frankfort.

The current state fees are:

Birth Certificate: $10
Death Certificate: $6
Marriage Certificate: $6
Divorce Certificate: $6

*These fees are non-refundable search fees.

The Kentucky Office of Vital Statistics maintains:

Birth records: 1911 to present
Death records: 1911 to present
Marriage records: June 1958 to present
Divorce records: June 1958 to present

For marriage records prior to June 1958, contact the County Clerk in the county where the marriage license was issued. For divorce records prior to June 1958, contact the Circuit Court Clerk in the county where the divorce decree was granted.

How to Order a Certificate

Certified records may be ordered directly from the Kentucky Office of Vital Statistics.

Orders may be submitted:

By mail
In person at the Office of Vital Statistics in Frankfort
Using the OVS drop box in Frankfort
Online, by telephone, or by fax through VitalChek, Kentucky's authorized vendor

The Office of Vital Statistics states that orders received by mail or through its drop box are generally filled within five business days, although some requests may take longer. For mailed requests, applicants should allow up to 30 working days for processing.

Home Birth Registration

Births occurring at home or in locations other than a regular birthing hospital must be registered with the Kentucky Office of Vital Statistics.

Depending on the credentials of the person who attended the delivery, registration may be completed electronically through KY-CHILD or by submitting the required forms and supporting documentation to OVS.
